Use this form to pre-register for a Teen Card from Huntington Beach Public Library.

- You must be 13-17 to get a Teen Card.
- You must fill in this form by 3 PM on Monday, May 13th.
- Make sure you have a photo ID (student ID is fine) on Thursday, May 16th. A representative of Huntington Beach Public Library will be on site on to hand out the cards.
- If you already have a children's card, you can change to a Teen Card. You will only be able to check out 5 items at a time with the teen card, but you will not have overdue fines. You can check out 25 with a children's card.
-If you have fines on your current children's card, you will need to pay them off at a HBPL branch before we can issue a Teen Card.
- If you do not know if you have a current card or have fines, please sign up anyway and we will check for you!
